The patient’s provider can bill CMS monthly for the time our care team spent managing the patient's conditions. We provide a guaranteed compliant billing process. Through our CCM program patients receive monthly care via telephone calls. These calls provide assistance and advice to patients about medication management, preventive care and increase patient awareness and involvement. A comprehensive care plan is provided each month to the participating physician. We also coordinate the scheduling of the patient’s Annual Wellness Visit.
